Release date: 2007-12-04 Renal protection of losartan in Japanese patients with type 2 diabetes A previous study, RENNAL, showed that patients with type 2 diabetes and kidney disease benefited significantly from losartan treatment. Of the 1,513 patients enrolled in the study, 96 were Japanese. Japanese researchers recently conducted a double-blind, randomized, subgroup study of 96 patients comparing the efficacy of 50 mg/d losartan, 100 mg/d losartan, and placebo in the Japanese population. All patients were given losartan or placebo on the basis of continued routine blood pressure lowering therapy, with an average follow-up of 2.8 years. The primary end point is a doubling of creatinine, end-stage renal disease or death. The secondary endpoint is the change in proteinuria. The safety of the drug was also evaluated. The primary endpoints of the losartan and placebo groups were 50% and 65.4%, respectively. After adjusting for differences in proteinuria at baseline, the efficacy of losartan was more pronounced (relative risk decreased by 45%, P = 0.0397), and the efficacy was independent of the antihypertensive effect of losartan. Compared with the placebo group, losartan reduced the effect of proteinuria (P < 0.001). Patients were well tolerated to losartan, both in the Japanese subgroup and in the entire patient population. The researchers pointed out that for Japanese patients with type 2 diabetes and kidney disease, losartan has a protective effect on the kidneys and is well tolerated.
